Now booking Phase 2B eligible Albertans for vaccinations.

Service Description

Phase 2B eligible Albertans will be able to make appointments though Penny Faus immunizing with Pfizer or Moderna vaccine or through AHS. Albertans who meet the following criteria will be able to book appointments: • Albertans born in 1973 or earlier who have a chronic medical condition. • First Nations, Metis or Inuit (FNMI) people born in 1981 or earlier. All eligible age groups, no matter the birth year: Phase 1: Born 1946 or earlier Phase 2A: Born 1947-56, FNMI born 1962-71 Chronic Medical Conditions: Albertans will self-identify to the pharmacy that they are eligible to be immunized if they fall within the list of eligible chronic conditions. You can view this list at https://www.alberta.ca/assets/documents/covid19-vaccine-phase-2B-eligibility-fact-sheet.pdf. Pharmacies are NOT expected to validate the presence of a chronic condition for eligibility in this category. We will be operating on the honor system where patients will be asked to attest that they have one or more of the eligible conditions. However, Albertans may want to talk to you to understand if their condition is on this list. FNMI (First Nations, Métis, Inuit): The booking of appointments will be based on the self-identification of the individual. At the time of the appointment, only identification that includes the individual’s birth date will be required. Examples include driver’s licence, passport, Alberta Health Care card or FNMI status or identification card. As a reminder, if a pharmacy receives a request for an appointment from an Albertan that was eligible in Phase 1A, 1B or 2A, priority should be given to these Albertans, as these individuals remain at greatest risk. Alberta Health Services is also accepting appointment bookings for Phase 2B. As with previous phases, proof of age and Personal Health Number are required to be presented to the pharmacy at the time of appointment. In order for the pharmacy to ensure eligibility for the COVID-19 vaccine, please see the following information regarding FNMI individuals and staff of licensed supportive living facilities.
